Dropdown widget disappearing in test mode

I am having an issue with a dropdown widget disappearing in test mode. Image of test mode below. The widget only disappears when a backend query is added to the dropdown for a specific table. The table has the structure of country (string), and state (string/list)

What have you tried so far?

Have tried rebuilding the page, copying existing dropdown which appearing on screen, tried different tables to see what effect that has. Tried looking in community and googling issue and looking on youtube.
