Not Able to use dropdown

Hi ,

Can someone tell me why I ma not able to full list of drown . only first row 's dropdown is working only . After that dropdown is not working .
Also I am trying to update drop down list , same data is not collecting in data collection . It is collecting "true" and "true" .

What have you tried so far?

I tried to change widget and variable also .
