Workflow for selecting a container, updating state of the container and passing a text field within the container as a variable to an AppState variable

Hello!

I am trying to create a questionnaire flow (please see example picture below) where the user can select a custom-created container. We need to change the state of that container to fill in the circle icon at right, and then pass the text within the selected container to an AppState variable for temporary local storage.

Any suggestion on how this could be done?
